Case management processes and software need to capture a good balance of structured and unstructured data without overwhelming the service user, their parents or carers, and the practitioners required to add it to their files or case management systems.
EO uses the ‘RAPIERE’ Case Management framework, which Jes originally developed during his time in practice, to facilitate discussion about each stage of a service user’s journey in their work with service providers from referral to exit.
We have a particular interest in the referral stage of case management, which is often overlooked in terms of its importance, particularly when studying demand from a systems-thinking perspective.
